Abstract
The study group for "Continuing Education and Quality Improvement" of the Association
of Cooperative Centres for Rheumatology in the Deutsche Gesellschaft für Rheumatologie
(DGRh) has organised an educational programme for general practitioners (GPs). The
educational programme is presently being evaluated by a study group supported by the
German Ministry of Health. It is a programme based on andragogic principles for interactive
learning in small groups, provided for use in the ongoing network of Audit Circles
for GPs in the BRD. The programme has been adopted from the project "Rheuma 2000"
of the Swiss League against Rheumatic diseases. In collaboration with the KV Hessen
and the KV Lower Saxony, a study group of the Rhein-Main and Hannover Centre for Rheumatology
and the Scientific Institute of the German Medical Association (WIAD) are presently
evaluating scientifically the educational programme in 2 Audit Circles each in Hessen
and in Lower Saxony. The evaluation includes acceptance of the programme (Topic: shoulder-neck
pain) by the GPs, increase in specific knowledge and outcome in respect of changes
in diagnostic and therapeutic policy of audit participants. Preliminary investigations
with the evaluation questionnaires in 6 pilot audit circles demonstrated that (1)
the novel didactic concept was excellently accepted by the participating GPs, (2)
that there was a measurable increase in learning according to the topic "shoulder-neck
pain" and that there are (3) changes in diagnostic and therapeutic policy towards
an improvement of patient medical care. Interactive, case-oriented learning in small
groups like Audit Circles of GPs with adaptation to adult learners seems to be an
alternative to formal lectures in continuing rheumatological education and may also
contribute to better medical care of patients with rheumatic diseases.
Zusammenfassung
Die Projektgruppe ,,Fortbildung und Qualitätssicherung" der Arbeitsgemeinschaft Regionaler
Kooperativer Rheumazentren in der Deutschen Gesellschaft für Rheumatologie (DGRH)
hat ein Fortbildungskonzept für niedergelassene Hausärzte entwickelt, das gegenwärtig
in einem BMG-geförderten Projekt evaluiert wird. Das nach Prinzipien der Erwachsenenbildung
(Andragogik) konzipierte Fortbildungsprogramm ist für den Einsatz im Netzwerk der
zur Zeit entstehenden Qualitätszirkel vorgesehen. Das Fortbildungskonzept hat die
bereits bewährte Schweizer Fortbildungsreihe ,,Rheuma 2000" zum Vorbild. In Kooperation
mit der KV Hessen und der KV Niedersachsen erfolgt derzeit unter Federführung der
Rheumazentren Rhein-Main und Hannover sowie unter wissenschaftlicher Begleitung durch
das Wissenschafliche Institut der Ärzte Deutschlands (WIAD) die modellhafte Evaluation
in je 2 Qualitätszirkeln in Hessen und Niedersachsen. Geprüft werden die Akzeptanz
des Fortbildungskonzeptes durch die teilnehmenden Ärzte, der vermittelte Wissenszuwachs
und die Auswirkung der behandelten Inhalte auf das ärztliche Verhalten. Modellthema
der rheumaspezifischen Qualitätszirkel ist der ,,Schulter-/Nackenschmerz". Ein vorläufiger
Einsatz der Evaluationsinstrumente (Fragebogen) in 6 Pilot-Qualitätszirkeln hat bereits
eine generell sehr gute Akzeptanz des neuen didaktischen Konzeptes, einen meßbaren
Wissenszuwachs und eine die Versorgungsqualität verbessernde ärztliche Verhaltensänderung
ergeben. Die bisherigen Ergebnisse bestätigen die bereits in der Schweiz und der BRD
gemachte Erfahrung, daß das Lernen in interaktiven Gruppen (Qualitätszirkeln) unter
Einbringung eigener Probleme die traditionelle ärztliche Frontalfortbildung in effektiver
Weise ergänzen - und zugleich zur Verbesserung der Versorgungsqualität rheumakranker
Patienten beitragen kann.
